Student Device Request Form for Student Access to Enrichment Resources

During school closures, LEUSD will loan out a student device for at home use and access to enrichment resources online (one device per household).

Please note, for a student device to work you must be able to provide your own Internet Hot Spot or home Internet access.

  • To request a device for your child, please complete and submit the online form linked below by Friday, April 3, 2020. Additional information is included on the online form, so be sure to open and read the form before you submit a request.
  • Once a submittal is approved, your child’s school will call to set up an appointment to pick up the device from school.
